- tailing off on power, especially uphill

That is not evidence by itself. It is a fair assumption the filter is full once the light is showing too.
Why diesel particulate filters clog near Red Scar

The first scan is there for exactly that. If it proves to be one of those, you are told before any work begins rather than after you have settled up a penny.
Gutting the filter out is not a remedy: without a serviceable DPF a vehicle will fail its MOT, and driving it is illegal. DPF renewal is the wording people look up, though nothing is swapped — your own filter is cleared in place and returned into use. Two more worth noting.
