- the wrong figure from a temperature sensor
- the EGR valve held open
- a fault in the AdBlue system
- faulty glow plugs or injectors, so never a regeneration ever runs
- the turbo passing oil into the filter
The first diagnostic is there for precisely that. You are told before the work gets going, not after you have paid, if it turns out to be one of those.
Two things more worth knowing. Leaving the filter gutted is not a remedy — a vehicle without a serviceable DPF cannot pass its MOT and driving it is illegal. And cleaning is sometimes described as DPF renewal: nothing is exchanged, your own filter is treated in place and returned into use.
